DDD Support Coordination
Jewish Family Service offers support coordination for individuals with developmental disabilities. Our DDD (Division of Developmental Disabilities) team works to evaluate the need for services such as medical support, vocational and employment services, day programs, residential facilities, and social and educational opportunities and link the individual to those services. The team reflects on each individual’s strengths, aspirations and goals and matches their goals and aspiration to concrete resources. Support coordinators help manage the budget allocated by the state for the purchase of supports and services.
